The Early Intervention in Psychosis Outpatient Service and the First Episode Psychosis Network of Eginition Hospital in Athens

1. Purpose: To present the development of Early Intervention in Psychosis (EIP) unit into a First Episode Psychosis (FEP) network. 2. Materials and Methods: Eginition University Hospital (EUH) in Athens has been addressing Early Psychosis through a specialized outpatient EIP service since 2012, providing care for both At-Risk-Mental-State (ARMS) and FEP subjects. The main assessment tools used are Comprehensive Assessment of At-Risk Mental State (CAARMS) and Nottingham Onset Schedule (NOS). Since EUH had been providing standard care for FEP, the clinical focus of the EIP service was mainly towards ARMS. During the last 12 months a network of directly linked services for FEP has been created, involving the EIP service, an Inpatient Unit for prompt hospitalization and a Day Clinic for partial hospitalization. 3. Results: From 3/2012 to 3/2015 the EIP unit received 26 (40%) ARMS and 17 (29%) FEP out of 65 subjects. Until March 2016 the referrals rose to 92 with 29 (32%) ARMS and 36 (39%) FEP. During the first 3 years the transition rate to psychosis of ARMS was 19.2% and the relapse rate of FEP was 11.7%. From the new subjects that entered the EIP unit during the last year none of the ARMS made a transition and only one of the FEP had a relapse. 4. Conclusion: The integration of an EIP service into a FEP network in Athens has cohesively enabled a broader therapeutic framework, shifting the clinical focus towards FEP, although ARMS subjects are still assessed and treated by the EIP service.
